You scanned the code on a piece of RTMP equipment. Tell us what is wrong with it so it does not go back out on the next show.

Fill out the form and we will pull the item from service. If the gear is needed for a show that is already loading in, call the production line as well so somebody knows right away.
- What the item is, plus any asset or case number on the label
- Where it is right now — venue, room, shop, or which truck
- What is wrong, and whether it still powers on
- Whether it is needed for an upcoming show
- Your name, so we can follow up if we have questions
